Sliding Scale

The fee is differed by the severity of problems, frequency of treatment, and client's financial stability. I offer a 45-60 minute consultation with a flat fee of 275.  Feel free to email me if you have any questions regarding services.

Payment Method

By Credit cards using a HIPPA Compliant Payment APP.


Cancellation

Cancellation < 12 hours: 100% of the fee will be charged.

Cancellation < 24 hours: 50% of the fee will be charged.

Cancellation < 48 hours: 20% of the fee will be charged.

Cancellation > 48 hours: No Charge

  • Provider’s name and license information

  • Diagnosis and procedure codes
  • Fee for the service

  • Log in to your insurance company's website or use their mobile app to submit a claim.
  • Attach the superbill and any necessary forms.
  • Insurance providers may also allow submission via mail or fax.

5. Receive Your Reimbursement 

  • If approved, your insurance provider will reimburse you based on your plan’s coverage.
  • This amount may be sent via direct deposit or check, depending on your insurance provider.
  • Processing times vary, but reimbursement can take anywhere from a few weeks to a couple of months.
